At DFSLS, we understand that support needs don’t always follow a schedule. That’s why we have a 24-hour response plan in place to ensure help is always just a phone call away.

In the event of an emergency, our response staff can securely access a client’s information through a mobile app, including essential documents such as the Individual Program Plan (IPP), Individual Service Plan (ISP), medication records, and insurance details. This supports timely, well-informed care and peace of mind for both clients and families.

Home of One’s Own
- Individuals live in homes that they own, lease, or rent like other members of their community.
- They choose where they live, with whom, and they control what happens in their home.
- Individuals are secure in their homes and do not have to move if their needs, services, or service agency change.
Individuals are safe in their home and neighborhood.
Choice and Self-Directed
- The individual makes their own everyday choices.
- Individuals plan for their futures.
- They direct the services they receive and have a choice of agencies and staff.
- Individuals are supported (e.g., technology, communication devices, behavioral support) to communicate their preferences, choices, and needs.
- Individuals are satisfied with the services they receive.
How we will implement:
We build support around each individual’s goals and preferences, then adapt as needs change through consistent communication and collaboration.
Community Membership
- The individual participates in the mainstream of community life.
- Individuals have opportunities to join clubs, groups, and churches.
- Individuals use local community resources and generic services.
How we will implement:
We help identify community opportunities aligned with personal interests and provide support that removes barriers to participation and consistency.
Relationships
- Individuals have family, friends, and neighbors who support them in regular ways or as paid help.
- The individual and their circle of support work together as a team with the supported living agency and others to share responsibility for well-being.

We encourage strong support networks by promoting communication, trust, and collaboration across each person’s circle of support.
